Being well-prepared for you appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. It will also help relieve any unnecessary anxiety you may be feeling

Please remember to bring the following items to your appointment:

  • Health insurance cards

  • Photo ID

  • Your primary care doctor’s contact information

  • A list of any medications you are taking (pre scription and over-the counter)

  • A referral from your primary care doctor (if required by your insurance carrier)

  • Any previous x-rays, MRI’s other test results relating to any past foot health conditions

  • If you are a diabetic – It would be helpful to either bring or have your primary care physician fax me a copy of your most recent blood work.

Other Office Recommendation & Policies:

  • Remove all nail polish prior to your visit

  • Do not put any cream or ointments on your feet the day of your visit.

  • All minors (under 18) must be accompanied by an adult with authority to consent to the minor’s treatment.

Insurance and Payment Options

We will be more than happy to submit all insurance forms for you and help you recover the most from your benefits. We will do everything we can to help you afford the treatment you need and want.

For patients who require major work, a complete payment plan is designed with an appropriate payment schedule. If your insurance requires you to have a referral to visit a specialist, like HMO patients, please obtain one from your family practitioner before you come in to see us, and bring it with you to your appointment if applicable. If you do not, you may have to reschedule—or cover the cost of the visit yourself if insurance denies payment.

We will do everything we can to help you afford the treatment you need and want. For patients who require major work, a complete payment plan is designed with an appropriate payment schedule. Forms of payment accepted by the office are check, cash, or any major credit card.
